The Secure Screen Lock Out Policy is designed to protect sensitive information by ensuring that unattended computers automatically lock after a specified period of inactivity. The default inactivity period is set to 15 minutes, which is considered best practice for security. For specific clinical use cases, an extended lock out period of up to 30 minutes may be applied. Requests for periods beyond 30 minutes must be formally submitted and approved.

Screen Lock Out Override Request Form

This form is used by staff to request an extension of the screen lock out period beyond the default 15 minutes, up to 30 minutes for clinical use, or beyond 30 minutes in exceptional cases. The form requires the requestor to provide justification for the extension, describe the impact on workflows, and outline security measures to be implemented. The request must be reviewed and approved by the Doctor Partner/Responsible Party, IT Leadership, and COO.

Screen Lock Out Policy Acknowledgment Form

This form is used to ensure that all staff members have read, understood, and agreed to comply with the Secure Screen Lock Out Policy. The employee signs the form to acknowledge their understanding and agreement, and the form is then confirmed by their manager and processed by Human Resources. Non-compliance with the policy can result in disciplinary action, up to and including termination of employment.

These documents collectively ensure that the screen lock out policy is communicated, understood, and enforced across the organization, thereby enhancing the security of sensitive information.
